Medical Social Workers at the VNH are integral members of a multi-disciplinary health care team that provides skilled nursing and rehabilitative care to patients, allowing them to receive the medical care required in the comfort of their own home. 

The Medical Social Worker will:

  • Provide clinical social evaluations and plan appropriate interventions based on evaluation findings.
  • Identify appropriate community resource referrals to address the client's and the family's practical and environmental needs.
  • Act as an advocate for both the client and the family, to navigate the patient through the community system.
  • Participate in the development of the total plan of care with the multidisciplinary team of VNH clinicians, as appropriate.
  • Develop and maintain contracts with public and private agencies as resources for the patient. 

Minimum Qualifications: 

  • Prior home care experience is preferred.
  • Master of Social Work (MSW) required.

Seeking Licensed Mental Health Counselors/School Social Workers to work within a therapeutic day placement. Seacoast Learning Collaborative is an out of district special education day placement for students with academic and behavioral challenges, social skills deficits and mental health needs.

We are seeking individuals who have a passion for working with at risk youth and are strong team players with solid communication skills. Clinicians have small caseloads and opportunities to provide individualized programming. 

SLC has an adventure based programming component including an indoor climbing wall, music therapy. pet therapy which includes a comfort dog who works in conjunction with our SRO and two therapy dogs who visit weekly with our yoga/mindfulness instructor. Clinicians work in conjunction with teachers, para-professionals, related service providers and placing agencies, as well as families and community liaisons.

Master’s Degree in counseling or Social Work required.
