The New Hampshire Professionals Health Program (www.NHPHP.org) seeks a highly motivated, and team-focused Clinical Case Manager (CCM).

The role of the Clinical Case Manger is to provide primary interface between the Program, the participants and their collateral treatment providers, as well to provide case management services and documentation.

The CCM is dedicated to the success of our participants to practice safely under their license. For those participants who are monitored by the NHPHP, this requires that they meet all stipulations of their monitoring agreement.

The following are favorable criteria for candidates:

  • Hold an active license in the state of NH in any of the following areas: Psychology, Mental Health, Social Work or MSN with nurse case management specialization.
  • Experience in the mental health and addiction fields.

Trauma-Informed Clinician at HAVEN

This position will be a vital addition to the HAVEN supportive direct client services program, providing trauma-informed, empowering, non-judgmental, compassionate, and culturally sensitive therapeutic counselling and advocacy for individuals impacted by interpersonal violence from a social justice and equity rooted framework. The position will also provide guidance to direct service staff for peer support and processing vicarious trauma.

RESPONSIBILITIES:

  • Develop individualized trauma informed, victim centered and culturally sensitive therapeutic short term treatment plans that support self-sufficiency and refer out to a higher level of care as appropriate.
  • Maintain positive therapeutic rapport with clients, providing coaching and feedback to clients to promote healthy coping mechanisms, boundaries, and empowerment.
  • Conduct outreach with external mental health partners and community resources to facilitate referrals and multi-disciplinary team approach for care and treatment of clients as appropriate.
  • Provide client services, including coverage of the agency’s 24-hour hotline, providing services to walk-in’s during office hours and other responsibilities as needed.
  • Effectively assess and manage crisis situations to ensure the safety and well-being of clients and staff in a calm and competent manner.
  • Address behavioral health concerns through evidence-based interventions and experience in techniques to help clients modify negative thoughts and behaviors.
  • Conduct both individual and group counseling sessions to address client needs, as well as the facilitation of support groups.
  • Balance scheduled client appointments, ongoing case management and real time unscheduled client needs that arise during the provision of crisis services.
  • Deliver ongoing training and presentations internally within the organization to assist direct service providers in strategies and techniques for best practice in advocacy of survivors.
  • Provide guidance to direct service staff for peer support and processing vicarious trauma.
  • Function as a resource to the rest of the team on mental health related advocacy and assist in the various components of the client services program.

QUALIFICATIONS:

Education

  • Master’s degree required from accredited institution in Clinical Mental Health Counseling, Clinical Social Work, Psychology or related field required.
  • NH Licensure is preferred in Clinical Mental Health Counselling or Clinical Social Work but will consider pre-license candidates.

Skills/Knowledge

  • Familiarity with working in a strength based, trauma informed environment.
  • Knowledge of the dynamics of domestic and sexual violence and experience with individuals impacted.
  • Understanding of professional boundaries and ability to set appropriate limits.
  • Strong verbal and written communication.
  • Ability to work as both an effective team member and independently.
  • Knowledge of appropriate community resources and ability to work collaboratively with them to provide appropriate wrap around services and referrals.

Position Summary

This mental health counselor or social worker provides counseling and therapist services to the New Hampshire Army National Guard's (NHARNG) to support mental fitness and wellness for operational readiness. This is accomplished by assessing, identifying and resolving soldiers’ behavioral health concerns and by providing clinical oversight and coordination for the behavioral health components of the psychiatric, family, social, spiritual, and emotional support programs available to service members and their families.

The purpose of this position is to provide counseling and therapy services that are crucial to identifying Soldiers with behavioral health concerns and assisting in supporting the mental health of these Soldiers. The period of performance for this assignment will be on or until May 31st 2026. Hours of work shall be 8:00 - 4:00 Monday-Friday with one weekend a month during drill weekends. Some telework is authorized.

This position shall travel to National Guard facilities throughout the state of New Hampshire. Travel costs shall be borne by the government.

Special Qualifications:

  • Valid license issued by the NH Board of Mental Health Practitioners
  • One (1) years’ experience post-licensing
  • Possess Master’s degree in social work or clinical mental health counseling
  • Minimum of two years of experience providing mental health services in a supervised setting. Including a range of experiences involving adult patients, military couple’s therapy, PTSD and combat stress, depression, anxiety, adjustment disorder, suicide and crisis intervention, and substance abuse.

Specific Job Duties:

  • Assist in identifying problems and gaps in service in the current behavior health delivery system and devise methods to improve and enhance the system.
  • Maintain a case load of 30-40 cases.
  • Perform initial/intake assessments for self-referrals. The contractor shall make provisional diagnoses, formulate treatment plans, and provide follow-up to soldiers who access behavioral health services of their own accord.
  • Conduct routine mental health assessments with a focus on soldiers who have recently returned from deployment. The contractor will work with those individuals identified as having mental health issues by making provisional diagnosis, formulating treatment plans, and providing all necessary follow up in accordance with all applicable military regulations to include AR 40-501. In the process, the contractor shall facilitate the processing of Soldiers who are no longer fit for duty through appropriate medical and/or administrative separations.
  • Perform mental health assessments on Soldiers who have been command-directed to seek mental health services. The contractor shall support these individuals by making a provisional diagnosis, formulating treatment plans, and providing all necessary follow up in accordance with all applicable military regulations to include AR 40-501.
  • Advise Director of Psychological Health leadership on behavior health issues and trends with the potential to impact mission readiness. In the event of a case with high visibility or resource intensive requirements, the Contractor shall communicate these cases to the Director of Psychological Health within 24 hours of identification.
  • Be prepared to travel to various National Guard locations around the state to conduct assessments, consult with leadership, and provide education about preventative health topics. The contractor shall engage in wellness promotion focused activities and events in order to facilitate the prevention and education of mental health issues, early intervention, crisis response, and post-intervention.

  • Specialization: At least one niche market (e.g., children, teens, trauma, anxiety, grief, CBT).
  • Insurance Approval: Current approval as an in-network provider with UBH/Optum and/or Cigna.

Work Hours and Location:

  • Flexibility: Work hours may vary based on client and company needs
  • Location: Sessions will be held at Aloft Integrated Wellness, 21 Hampton Rd, Suite 104, Exeter, NH 03833, or via secure telehealth as agreed upon.

Compensation and Benefits:

  • Hourly rate plus bi-monthly commission
